Variety - The Children's Charity’s Freedom and Caring for Kids Program provides equipment to enable children to participate in their communities. We provide the following: enabling equipment (wheelchairs, adaptive car seats & strollers, and adaptive mobility bikes) and assistive technology and communication devices when this equipment cannot be obtained from any other source.

You will need the following items to complete your application:
• Medical Justification letter from a physical, occupational, speech therapist, or a doctor in the appropriate field.
• Income Verification: Copy of Tax refund. (Please make sure the social security numbers are NOT visible)
• Insurance denial letter if appropriate
• Vendors quotes for the equipment.
• Signature Release of Liability
• Signature of Disclaimer
• Signature of Authorization to use name and likeness
